Healthcare innovation is advancing rapidly. Artificial intelligence, digital health, remote monitoring, advanced medical devices and data-driven infrastructures are transforming how diseases are prevented, diagnosed, treated and managed. Yet technological performance alone does not guarantee meaningful clinical impact.
This narrative review examines a critical question: what does it take to translate healthcare innovation into routine clinical practice? Looking particularly at the European and Spanish contexts, the authors explore the regulatory, organisational, financial, ethical and governance challenges that can prevent promising technologies from progressing beyond controlled or pilot settings.
The review highlights key barriers across the innovation lifecycle, including complex regulatory pathways, limited interoperability, technology-transfer gaps, insufficient implementation-oriented funding, workforce adaptation, cybersecurity, data governance and unclear accountability for AI-supported decisions. These challenges become increasingly important as healthcare technologies—and particularly AI systems—become more autonomous and deeply integrated into clinical workflows.
The central message is clear: sustainable healthcare innovation is a system-level challenge, not simply a technological one. Real-world impact requires coordinated regulation, clinical validation, institutional readiness, professional oversight and long-term governance to ensure that innovation can be implemented safely, equitably and at scale.
